How to use the wells pe score

Tick each finding present. Original Wells: ≤4 unlikely; >4 likely. Three-tier: <2 low / 2-6 moderate / >6 high.

Formula & explanation

Weighted sum of clinical features. Used with D-dimer to rule out PE in low-probability patients.

Examples

HR > 100 + previous DVT + hemoptysis: 1.5 + 1.5 + 1 = 4 (moderate).

Frequently asked questions

What about PERC?
PERC is a separate rule for ruling out PE in low-probability patients without further testing — use it after Wells when probability is low.
